Explanation

Shine the penlight in every orifice: eyes, nose, mouth, in AND behind the ears. Are the pupils equal and reactive? Is there blood or other fluid coming from the nose or ears? Do we have battle signs behind the ears (bruising)? Is the mouth missing teeth or have other trauma?

Palpate the cervical spine for step offs/pain response, come around to the front of the neck and asses (0-4) for stoma, tracheal deviation, JVD, medical alert, subcutaneous emphysema.

Walk your fingers across both clavicles feeling for crepitus/instability, starting at the shoulders and working your way in towards the sternum.

Walk your fingers down the sternum, again for any crepitus/instability. Then slide hands to opposite sides of chest and give a firm push inwards, checking for rib/chest wall stability. Then asses breath sounds and observe for unequal chest rise/fall

While palpating, you're assessing for rigidity, distention, discoloration, and pulsating masses.

Slide hands out to iliac crests (pelvic wings) and push firm pressure down and release. Then place your hands where the greater trochanters are (so hands move more to the outsides and drop down about an inch or so, think where the legs meets the rest of the pelvis) and firm pressure in. These are checking for instability/crepitus. Also visualize any loss of bowel or bladder. And then visualize priapism in men (uncontrolled erection that can occur with a spinal cord injury).

You can place one hand on the outside of the upper thigh and the other hand on the inside of the lower thigh and apply counter pressure. Should be able to note any instability/crepitus. Hands can be reversed, just ensure counter pressure.

Find the patella (kneecap) and just ensure it's not floating off in space

Apply counter pressure, again checking for instability/crepitus

Asses for pulse, motor, and sensory (the last two if you're able/patient is responsive enough). Also called neurovasculars.

Just like the lower extremities, apply counterpressure to identify instability/crepitus.

Log roll the patient while maintaining c-spine and feel for step offs down the spine. Check once more for loss of bowel or bladder.
