Explanation

Yes! The elevated platform is not barricaded hence there is a danger of falling object to anyone below.

Yes! A hole where a person can fall into is a work-at-height hazard.

Yes! Loosely secured chain for hoisting is a falling object hazard. A better option would be using a textile sling / belt which can conform to the shape of the hoisted object for better securing.

Yes! All workers working-at-height must wear a helmet to prevent / minimised injury in the case of a falling from height or falling object hazard.

No! A warning sign itself is not a hazard, but in this case of inappropriate placing, it doesn't enable anyone to notice it to warn / remind of any danger.
