The product is missing part or all of the required ingredients. (i.e. garnish, solid pieces, noodles, chocolate chips, vegetables, etc.) OPTAVIA/Medifast bars were melted upon receipt Any damage to the product box or packaging. (i.e. ripped box, damaged bag or packaging, etc.) Client/Customer received a different product instead of what was ordered. (i.e. ordered Golden Pancakes, but received Chocolate Chip Pancakes) The incorrect number of products shipped versus products ordered. (i.e. the Client/Customer ordered 22 boxes of Fuelings but received 18 boxes)
