Troponins are structural proteins involved in cardiac muscle contraction (part of the actin–myosin complex). When myocardial cells die, Troponin I and T leak into the blood. They’re highly specific to cardiac tissue and remain elevated for days, making them the most reliable indicator of myocardial infarction.

Troponin regulates the interaction between actin and myosin in muscle fibres. It has three subunits — C (binds calcium), I (inhibits binding), and T (binds tropomyosin). When cardiac cells are damaged, the I and T subunits are released, which is why they’re measured in diagnostic blood tests.

Troponins are not visible immediately but begin to increase 4–12 hours after cardiac injury, peak at 12–48 hours, and can remain elevated for up to 15 days. This timing allows detection even if the patient presents late after symptom onset.

CK-MB (Creatine Kinase–Myocardial Band) rises 3–12 hours after infarction and returns to normal within 2–3 days, much faster than troponin. This shorter window makes it useful for identifying a second infarction occurring soon after the first, since troponin might still be elevated from the initial event.

Myoglobin is the earliest marker to rise after myocardial injury — appearing within 1–4 hours, peaking at around 6–7 hours, and returning to normal within 24 hours.

Troponin T persists in circulation for 5–15 days after infarction. This long duration makes it ideal for confirming an earlier cardiac event, even if the patient presents well after symptoms started.

While myoglobin rises very early, it’s present in all muscle types — not just the heart. Exercise, trauma, or muscle disease can also increase its levels, so it must be interpreted alongside more specific markers like troponin.

IMA appears within minutes of cardiac ischaemia, peaks around 2–4 hours, and disappears within 6 hours. It’s being evaluated as a very early indicator of cardiac ischaemia, before troponin levels rise

Troponin is a structural protein of the actin–myosin complex in cardiac myofibrils. When myocyte membranes become leaky during necrosis, troponins I and T escape into circulation — the biochemical basis for their diagnostic use.

An ideal cardiac marker should have a high concentration in heart muscle and be absent from other tissues, so that its release clearly indicates cardiac injury. It should also have a rapid rise, long enough half-life, low cost, and clinical usefulness (helps guide therapy).
