Product packaging appears punctured or torn. The product may or may not have leaked out.

OPTAVIA/Medifast bars were melted upon receipt

The incorrect number of products shipped versus products ordered. (i.e. the Client/Customer ordered 22 boxes of Fuelings but received 18 boxes)
